 Hold imports, defaulting to hold for box sets on request 
A "held" import status keeps a requested book downloading but out of the
importer until it's resumed. Requests can start on hold; the library
picker checks it by default when the title looks like several books
(rss.IsMultiBook), so box sets can be split before uploading.
